Notes:  From June 2009 the Western Australian Ombudsman’s office has been tasked with reviewing certain child deaths, identifying patterns and trends uncovered in the review process and making recommendations for improvement in Department of Child Protection practices to prevent or reduce child deaths.

From July 2012 the Ombudsman’s office will take on the task of reviewing family and domestic violence fatalities and during the 2011-2012 year covered in this report, considerable work was done in preparation for this expanded role.
