Notes:  Prepared by the Victorian Child Death Review Committee (VCDRC), an independent body established to provide external overviews of the findings of Child Death Inquiries, this annual report presents a background to the VCDRC, the Child Death Inquiries reviewed and the key trends revealed. Some of the major themes and issues that emerge from the review are then identified, including:
* cases involving infants with complex medical needs, particularly those whose parents are young, experiencing mental health issues and using drugs and alcohol
* adolescents with a long-standing involvement with the Child Protection Service (CPS) and the importance of long-term and continuous involvement of the CPS and of consultation and collaboration among service providers, including the Juvenile Justice service
* workload, staffing and staff supervision especially in rural areas
A brief summary of other activities conducted by the VCDRC is also included.
