Notes:  From June 2009 the Western Australian Ombudsman’s office has been tasked with reviewing certain child deaths, identifying patterns and trends uncovered in the review process and making recommendations for improvement in Department of Child Protection practices to prevent or reduce child deaths.

In July 2012, the Western Australian Ombudsman’s office began reviewing family and domestic violence fatalities in Western Australia. During the 2012-2013 period covered in this report, significant effort was put into the development of structures and processes to guarantee that these reviews are handled effectively and efficiently.
