Pursuant to a recently agreed mandate, this report presents the findings of the New South Wales Ombudsman’s first review of the deaths of children and people with disabilities, and makes a series of recommendations for reform to current State policy and practice with a view to preventing or reducing the incidence of untimely deaths. Figures suggest Indigenous children continue to be over-represented in child deaths in NSW, that Interagency Guidelines for Child Protection Intervention continue to be under-utilised and that there remains a need for improved coordination, planning and documentation of services provided to people with a disability.
